Men sleeping under six hours were more than four times as likely to develop a cold after direct viral exposure in a controlled study. A Las Vegas longevity expert breaks down the seven actual causes, the hygiene and environment fixes, and why the majority of a man’s testosterone is produced while he is unconscious.

LAS VEGAS, NV — Ask a man over 45 why he sleeps badly and he will usually say insomnia, stress, or that he has simply never been a good sleeper.

According to Dr. John Spencer Ellis, an internationally recognized authority on men’s health, longevity and human performance, the real causes are more specific than that — and most of them are fixable.

“Sleep is not one item on the list alongside training and nutrition,” said Dr. Ellis. “It is the multiplier on both. The research is unusually direct — the opening line of a JAMA paper on the subject states plainly that the majority of the daily testosterone release in men occurs during sleep. A man optimizing his training while sleeping six broken hours is renovating a house with a cracked foundation.”

The seven

1. Untreated obstructive sleep apnea. Common in men over 40, treatable, and diagnosed in a fraction of the men who have it. The same JAMA paper notes that sleep fragmentation and obstructive sleep apnea are associated with reduced testosterone. “If you snore or wake unrefreshed, get screened,” said Dr. Ellis. “No protocol overcomes it, and for some men that single step is the whole answer.”

2. Alcohol used as the off switch. It shortens the time to fall asleep and destroys sleep architecture — the opposite of what a man needs. “It is the most common sleep aid in America and among the worst.”

3. Chronic work stress. Cortisol and sleep form a closed loop: elevated cortisol fragments sleep, and fragmented sleep raises cortisol further. Neither resolves without deliberate intervention.

4. Nocturia from an enlarged prostate. Benign prostatic hyperplasia appears in roughly 27.8 percent of older men in prevalence data, and waking two or three times a night to urinate fractures sleep architecture regardless of time in bed. “This is a urologist conversation men delay for years.”

5. Late caffeine. With a five-to-six-hour half-life, a 3 p.m. coffee still has meaningful caffeine circulating at 9 p.m.

6. The weekend catch-up. Waking at 6:00 on weekdays and 9:00 at weekends produces the physiological equivalent of a three-hour time zone shift twice a week. The circadian system never settles.

7. Age itself. Deep sleep declines with age, so the same seven hours delivers less recovery at 55 than it did at 25 — and growth hormone release and the bulk of tissue repair occur during deep sleep.

Sleep hygiene, explained properly

“Sleep hygiene has become a phrase people nod at without knowing what it means,” said Dr. Ellis. “It is simply the set of behaviors that make sleep more likely, and it stops being optional after 45.”

Fix the wake time, not the bedtime. You cannot force sleep onset. You can set an alarm. A fixed wake time seven days a week — with a maximum one hour of weekend drift — pulls the bedtime into place within about a week, because sleep pressure builds on schedule.

Get morning daylight within an hour of waking, outdoors, no sunglasses. This sets the melatonin timing that makes tonight easier.

Aim for seven to nine hours in bed, counted as time in bed rather than time asleep. The gap between them is real and typically an hour or more.

Cap caffeine by early afternoon. No alcohol within three hours of bed.

Dim the lights and put screens down in the final hour.

The immune connection is not vague

This is where Dr. Ellis says the evidence is strongest and least appreciated.

Prather and colleagues, publishing in Sleep in 2015, tracked 164 healthy adults with wrist actigraphy for seven consecutive days, then quarantined them and administered rhinovirus nasal drops directly.

Compared with those sleeping more than seven hours, participants sleeping under five hours had an odds ratio of 4.50 for developing a clinical cold, and those sleeping five to six hours an odds ratio of 4.24. The association held independent of pre-challenge antibody levels, demographics, season, BMI, psychological variables and health practices.

“Everyone got the same virus, up the nose, under controlled conditions,” said Dr. Ellis. “The short sleepers were more than four times as likely to actually get sick. That is not a questionnaire finding. That is a controlled viral challenge.”

The vaccination literature points the same direction, with work by Spiegel, Sheridan and Van Cauter in JAMA, and by Lange and colleagues, showing sleep influences antibody response to immunization.

Cognition and appearance

Cognition. Overnight clearance of metabolic waste from brain tissue occurs predominantly during sleep. In Stanford research, athletes extended to ten hours in bed for five to seven weeks improved skilled performance measures by roughly 9 percent, with faster sprints and improved mood; a separate study found sleep extension improved reaction time by 15 percent.

“Men working seventy-hour weeks on six hours of sleep are making their most consequential decisions in their least capable state,” said Dr. Ellis.

Appearance. Sleep debt registers under the eyes within days. Elevated cortisol and alcohol drive facial fluid retention and inflammation. And dermal collagen — already declining roughly one percent per year after 30 — receives less repair support when deep sleep is compromised.

“Men come to me about how they look and stay for what the bloodwork does. The mirror is simply the part they check daily, and sleep is the fastest thing that moves it.”

The sleep environment

Dr. Ellis is specific about the room itself.

Cold. Core body temperature must fall to initiate and maintain sleep. Roughly 65 to 68°F suits most men, and a warm shower an hour before bed helps by driving peripheral heat loss afterward.

Dark. Genuinely dark — blackout treatment, and every standby LED covered or removed.

Quiet, or consistently masked with white or brown noise, which is preferable to intermittent silence broken by traffic.

The bed is for sleep. Not the laptop, not the phone, not the television. “You are training an association, and most men have trained the opposite one.”

Separate beds are not a failure. If a partner’s snoring or schedule is fragmenting your sleep several nights a week, that is a practical problem with a practical solution, and men should stop treating it as a verdict on the relationship.
